Have you experienced any of the following symptoms in the last 2 weeks?  If so, please seek medical attention immediately and as a precaution, refrain from coming in contact with others as much as possible.

• Shortness of Breath
• Cough
• Fever/Chills
• Sore Throat
• Nausea/Vomiting/Diarrhea
• Fatigue
• Exposure to anyone that has tested positive for COVID-19
